Buyer authorizes StressBusters to charge the credit card listed below, for the amount chosen above, on the date of Buyer's first service, and every month thereafter on the first day of each month. This contract is for a minimum period of six (6) months, with a month-to-month ongoing payment structure. Payments will continue after the six (6) month period, unless Buyer cancels this agreement per the cancellation policies listed below.

Additional Terms and Policies
Each month StressBusters will charge the amount listed above that was chosen by the Buyer.  After the charge is processed, StressBusters will credit Buyer with one (1) massage session to their account.
Buyer will be given the same discounted rate for as many massages that they want to get during the month.
Membership discounted prices may not be paid for with a gift certificate, nor SpaFinder, Spa Wish or Spa Week gift cards.

Membership discounted prices may not be combined with any other discounts offered.

Membership credit is transferrable but is non-refundable.
Cancellations, Modifications to Scheduled Appointments & No-shows
In order to serve as many valued guests as possible, and for the respect of our staff’s valuable time, the following policies apply if you ever need to cancel your scheduled appointment; move your scheduled appointment to a different day or you do not show up for your scheduled appointment.
Cancellations, modifications for any and all scheduled treatments, we require a 48-hour notice. Any changes received with less notice will be charged the full amount of the scheduled service to the Buyer. If we are able to fill the appointment however, you will NOT be charged.

In addition to the above, changes to scheduled appointments lasting more than 90 minutes (example: a 55 minute massage and a 60 minute facial combination), we require a 72-hour notice to avoid being charged. Again, if we are able to fill your appointment time, you will NOT be charged; otherwise the Buyer is responsible for the entire cost of the scheduled service.

If you do not show up for any scheduled appointment, you will be charged the entire amount of the scheduled treatment.
Membership Cancellations
Cancellations must be done in writing by sending an email to:  SBSchedule@StressBustersSpa.com
Less than six (6) months from sign up date
Option A:  You can choose to freeze your membership for a maximum of two (2) months.  The time that your membership is in "freeze" status will not count towards the six (6) month membership period.

Option B:  Cancel and pay the difference between full-priced services and the discounted price you received.  For example, say you received 3 discounted massages at $89 per session, but the full-price was $99.  We would charge you an additional $30 to make up for the discount that is only given to six (6) month members.  Furthermore, a $25 processing fee will be charged.
After the six (6) moth membership period
Membership may be cancelled with a thirty (30) day written notice.
